The most difficult medical challenge
for both doctor and patient alike is
the “c” word. No, not cancer.
Compliance: the not-so-simple task
of getting a patient to do or take what
is needed to maintain or salvage good health. A number of
years ago, for example, thirty-five thousand women articipated
in a study on the benefits of taking calcium tablets.
In the end, the most compelling finding was not about
bone mass density or weight loss or any of the other benefits
the researchers had set out to investigate–no, the most
compelling finding was that only 40% of the participants
even took the designated pills with enough regularity for
the results to be viable. In a similar vein, another study–
this one on the effects of omega-3 in pregnant women and
their babies– had to be cancelled when the scientists recognized
that ALL of the participants had stopped taking
the prescribed fish oil capsules or placebo pills a mere 2
months into their gestation-long commitment. And this
was not in the U.S.– where fish oil is still considered a bit,
well, “fishy” – but in Denmark, where cod liver oil is as
much a household staple as our own peanut butter.
It is estimated that only 50% of all American prescription
users follow (or remember to follow) their physicians
advice, a flaw in the system that leads to nearly $100 billion
in increased health costs and suffering per year.
This nonadherence
to medical regimens is called “non- compliance”
and it is considered by many to be the single greatest
obstacle to improving patient health. We’ve certainly seen
the problem in our own practice over the years, which is
what led us one day to this simple realization. “We can’t
help people lead healthier lives if we can’t improve their
rates of compliance.”
Once we committed ourselves to making compliance our
primary goal as physicians, our work began to crystallize:
first, there was the invention of a new medical device
